Your application should include a statement of no more than 250 words, illustrating how you have made a difference at CVHS. Whether through initiating a club, mentoring fellow students, demonstrating leadership, or contributing to the arts, surprise us with your unique and impactful contributions!

Additionally, please provide a comprehensive list of the activities and organizations in which you have been involved. Lastly, include the name and contact information of an adult reference who can speak to your achievements and character.
